APPLICATION AND CHECKLIST

PCOT membership process

Follow the 7-step process to become a member of the PCOT and gain access to the services that help your independent practice run more smoothly.

  1. Complete the application.
  2. Pay the non-refundable application fee of $350.
  3. Provide appropriate documentation for credentialing.
  4. Go through the full credentialing process.
  5. Be approved by the Credentialing Committee.
  6. Be approved by the Board of Directors.
  7. Become a PCOT member will full privileges.

Allied Health Professionals

$375/year

(affiliate non-voting members only)

Primary Care Physicians

$700/year (billed quarterly)

(Pediatrics, Internal Medicine, Family Practice, and General Practice)

Specialists

$1,100/year (billed quarterly)

Membership requirements

All applicants must meet the following requirements to become a member of PCOT:

  1. Hospital staff priverages: All applicants must have hospital privileges or a signed hospital waiver when applicable. Full inpatient, non-probationary staff privileges at the practitioner’s primary inpatient admitting JCAHO facility within the provider’s service area is otherwise required.
  2. Staff category changes: Any changes must be reported to the PCOT within 10 days. Hospital privileges must be maintained to participate in PCOT.
  3. Credentialing: An on-site office review is required for all members of PCOT. The credentialing process can take between 30-60 days barring any delays.  PCOT recredentials physicians in accordance with TDI and NCQA standards every 2 years.
  4. Payment: All physicians participating in the applicant’s group must be current with all payments to the PCOT. Membership will be revoked if payment to PCOT is 90 days past due.
